Software Evaluation Request

The Software Evaluation Request form is used when a campus is wanting to purchase or renew a software license.  The evaluation includes questions regarding software usage at the campus, if it is a renewal, and a few additional questions regarding the purchase.  

When completing the Software Evaluation Form, you will need to upload the software usage report (if a renewal), and the software quote.

To request that a software evaluation form be sent to you, click the link below and complete all required information.  Once you complete the request below, you will receive an email from Docusign with the evaluation form.
