Attached, you will find the necessary documentation required to process the disbursement to SLO Lead Teachers and teachers for the upcoming SLO training in August.

Please be advised that the documentation varies for each group of teachers (account numbers, composite form titles, and supporting documentation). Each form comes with its own set of instructions. Kindly ensure to thoroughly read and follow these instructions. Click on the provided links to download the appropriate forms.

The deadline for submitting the completed paperwork is Monday, August 19, 2024.

Please ensure that all paperwork is delivered to the HR Office.
